              All companies are searching for the next killer work model, app, platform, product, or idea. It’s too late when there are not enough products in the pipeline to create a sustainable competitive advantage. Companies learned this lesson the hard way.

Plant and equipment do not fail by accident. There are causes. Whether equipment fails depends on the capacity of their parts to handle stresses, when stresses are applied and the size of those stresses. These are probabilistic events – they are random, with many possibilities. Timing the start of failure, or its continuation to a breakdown, is mostly speculation because it depends on which past and future scenarios occur. It seems that luck and chance has a large say on the lifetime reliability of equipment. But there is a way to guide equipment reliability and performance toward the results you want.

Software reliability and hardware reliability are two distinct concepts within the field of engineering, each with its own unique characteristics and measurement challenges.

Software reliability is defined as the probability that software will operate without failure for a specified period of time in a specified environment. It is a reflection of the design perfection rather than manufacturing perfection, which is more associated with hardware reliability. The complexity of software is a major contributing factor to software reliability issues. Unlike hardware, software does not degrade over time or wear out, but it may have faults due to design defects that can cause failures.

Simple join colleagues for lunch and ask about the ‘major disasters’ of the past. The stories help us to remember and hopefully avoid repeating mistakes.

A Hybrid Methodology combining the speed and flexibility of Agile and the structure of Waterfall is feasible and practical. I base my opinion on the research I have done. I recently presented a Webinar to Project Management Institute (PMI) members on how to merge Agile and Waterfall to achieve a Hybrid methodology. The focus of this paper is to address the greatest risk to a company in implementing the new methodology i.e., acceptance of it.
