In-House or Contracted Oil Analysis with Bryan Johnson

We’re thrilled to have the chairman of ICML, Bryan Johnson, with us. He’s worked at the Palo Verde Generating Station for 30 nearly years as a lubrication engineer. Bryan’s also involved with ASTM, starting about three decades ago. That was due to the creation of an on-site oil testing lab. This became a subcommittee that deals only with the condition monitoring of lubricants.

In this episode, we covered:

  • What are the benefits of on-site oil analysis?
  • How do you know you’re getting consistent results from an analysis?
  • What are the standards to follow?
  • And much more!

5 Steps to Securing Commitment for a Transformer Reliability Program

5 Steps to Securing Commitment for a Transformer Reliability Program

by Alan Ross

When you start to put a risk and reliability program together for high voltage electrical equipment—primarily transformers—you have to go through a cultural change. I’ve seen it repeatedly, both in the industrial marketplace and the utility marketplace.

It’s never a one-person decision. [Read more…]

Never Let a Serious Crisis Go to Waste

Never Let a Serious Crisis Go to Waste

Guest Post by Joseph Paris (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

In an interview with the Wall Street Journal on November 19 ,2008, Rahm Emanuel was quoted as saying; “You never want a serious crisis to go to waste.  And what I mean by that [is] it’s an opportunity to do things that you think you could not before.”

He was referring to the crisis that was to become known as the “Great Recession” that gripped much of the world, hitting hardest from 2007-2008.  It was a period when millions of people lost their homes to foreclosure, nearly countless businesses declared bankruptcy; many being household names such as General Motors and Chrysler – along with a great number of financial institutions. What Managers May Not Know About Root Cause Analysis (RCA)

What Managers May Not Know About Root Cause Analysis (RCA)

Guest post by Mark Latino

If managers knew what the overall power of a well supported Root Cause Analysis (RCA) effort meant for their bottom-line, they would be breaking down doors to implement the process.

Unfortunately, this is often not the case, so this paper is an attempt to educate such individuals about the characteristics of an effective RCA methodology. The paper focuses on three aspects of RCA:

  1. What is RCA?
  2. What it takes to implement an effective RCA process as a way of conducting business rather than a finite ‘program’ that will eventually end
  3. How does RCA contribute to a company’s bottom-line? [Read more…]

ASQ Story: Coronavirus Forces Organizations to Adapt Business Models

ASQ Story: Coronavirus Forces Organizations to Adapt Business Models

Guest Post by James Kline (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

As readers of my pieces know, there are concerns about how ASQ is being managed.  These concerns have caused a split among the membership. Some hold fast and support corporate management, while others want changes. This piece is the result of a question and an exchange on the LinkedIn Restore ASQ community.

The original discussion question on Linkedin was: ‘Why didn’t the risk analysts see the pandemic coming’?  The exchange quickly devolved into those who felt that the disbanding of some ASQ Chapters is the fault of chapter members, not corporate and those who place ASQ’s troubles on corporate management. Considering the Coronavirus is forcing everyone to review assumptions, associations, and business models, it is worth visiting these issues. [Read more…]

Building a Risk Analysis Framework

Building a Risk Analysis Framework

Guest Post by Andrew Sheves (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

Your assessment needs a clear framework to help you process the information you have and analyze the risks. Sometimes you will have one prepared but at other times, you need to do this yourself. Whatever the situation, you need to know what goes into designing and building a framework if you’re going to be successful.

You will often find that the categories, methodology and metrics you’ll use to actually evaluate your risk are given to you or prepared in advance. You simply take the information you’ve already gathered and pass it through the system.

But what do you do if you don’t have a neatly laid out process? Or how can you spot where there’s a flaw in the methodology being used? [Read more…]

Solving Human-Caused Failure Problems

Solving Human-Caused Failure Problems

Guest post by Charles J. Latino

At the root of most mechanical and system failures lurks a human cause. Insights into what to look for when solving human-caused failures are essential. Human error is generally described as behavior that goes beyond the norm. A proper definition in the context of this article is, “an action planned but not carried out according to the plan”. To find a means of minimizing human error, one must first understand its characteristics: [Read more…]

Myth Busting 14: Integrating systems

Myth Busting 14: Integrating systems

In theory, integrated computer systems enable multiple uses for any single piece of data that is input only once. Data becomes available wherever it needs to be in whatever business process is integrated into the whole. In a sense it is like our brains – information and experience is registered once and available for access whenever needed for any purpose. Integrated systems should make our lives at work easier, but they seldom do that. Integrated business computer systems are very complex and can be very difficult to use. Imagine that they must support many separate business processes and departments, all of whom have different “languages” and ways of doing things. [Read more…]

What is the difference between a CAR and a PAR?

What is the difference between a CAR and a PAR?

One of the more confusing distinctions for quality professionals, especially those working within one of the quality management standards such as IATF 16949 or ISO 13485, is the difference between a Corrective Action Request (CAR) and a Preventive Action Request (PAR). [Read more…]

Combining CMMS with Remote Asset Monitoring for World-class Maintenance

Combining CMMS with Remote Asset Monitoring for World-class Maintenance

The biggest change in asset management practices and maintenance models in the coming years will be the shift from corrective and preventive to predictive and condition-based maintenance that is built on real-time as well as historical data.

With emerging new technologies like IoT (Internet of Things) and easy access to the Internet and cloud storage capabilities, accessibility to information and remote monitoring of assets can be done anytime and from any location and device. This calls for a reliable remote monitoring setup accompanied by a robust and proven management system to track and manage all that data. And it is this combination precisely that can elevate your business to a world-class maintenance program.

Myth Busting 13: We need our old data

Myth Busting 13: We need our old data

You’ve acquired and are now implementing a new CMMS / EAM (Computerized Maintenance Management System / Enterprise Asset Management) computer software program to help you manage maintenance. It may be a simple functional system that only looks after maintenance and likely Maintenance, Repair and Operating (MRO) spares, or it may be part of a much bigger enterprise system that handles many business functions. Regardless, one question almost always arises when converting from one to another system – what should we do about our old data? [Read more…]
